I chose to engage in this program and agree to do so at my own risk, of voluntarily participating in these activities, assume all risk of injury to myself, and agree to release and discharge Alexia Symes and Talia Savva Kukard from any and all claims or causes of action, known or unknown arising from The Pilates Course and their platform.

I hereby acknowledge this release, any representatives, agents and successors from liability for accidental injury or illness which I may incur as a result of participating in the said physical activity. I hereby assume all risks connected therewith and consent to participate in The Pilates Course with Alexia and Talia.

I agree to disclose and physical limitations, disabilities, ailments, or impairments which may affect my ability to participate in these classes.
